2. Photocopies (03 copies) of 12th Class Pass certificate & Marksheet.
3. Photocopies (03 copies) of Graduation year wise marksheets and pass certificate (for admission to P.G.).
4. Original Transfer Certificate/School Leaving Certificate.
5. Original Migration Certificate.
6. Original Character Certificate.
7. Photocopies (03 copies) of Certificate Claiming Reservation Benefits.
8. Photocopies (03 copies) of Certificate of Sports participation (at State/ University / National Level).
9. Medical Fitness Certificate from Registered Medical Practitioner or CMO.
10. 10 copies of Passport Size Photograph and 04 copies of Ticket size photograph.
11. 10 self addressed five rupees stamped envelopes of 11 cm by 25 cm.
12. Gap Certificate i.e. Regarding the reason of gap for the year / years of the gap that occurred.

Note :

1. All Certificates submitted as PHOTOCOPIES GOT TO BE VERIFIED AT THE ADMISSION CELL of the Institute at the time of counseling and joining the Institute. In case of any of the document lacking or the originals not verified at the time of admission, the admission shall be treated as provisional.
2. Photocopies (02 copies each) of all Original Documents should also be submitted with the form and also retained by the student for his / her own record.
